The Masimo NomoLine LH Adult Nasal CO2 Cannula with O2 (Mas-3820, 25 per box) Delivers supplemental oxygen to an adult patient and, at the same time, draws a continuous sidestream sample so your monitor can display an end-tidal CO2 number and waveform. - Low sampling flow, 50 sml/min. The ISA analyzer draws only 50 sml/min through this line, which is what lets it read accurately on patients with small tidal volumes and fast respiratory rates.3 Masimo specifies accurate measurement from 0 to 150 breaths per minute at that sampling rate.7
- Fast enough to trust the waveform. CO2 rise time at 50 sml/min is 200 milliseconds or less, and total analyzer system response time is under 3 seconds.3 Masimo states that under 3 second figure specifically with a 2 meter sampling line, which is exactly this part.7
- LH means short-term, low-humidity use. Masimo's directions for use define LH as designed for short term applications with low humidity, where the line collects water, and HH as engineered to remove aspirated and condensed water for long term, high-humidity applications.2 That is a duty-cycle distinction, not a room-climate one, and almost nobody publishes it correctly.
- Split-flow cannula with star-shaped O2 tubing. The split-flow design reduces mixing between the delivered oxygen and the sampled gas, and the star-shaped lumen on the oxygen tubing resists kinking.
- Connects straight to the ISA analyzer. Part 3820 is in Masimo's direct-connection group, so no Luer adapter and no multi-patient adapter is needed between the cannula and the analyzer.2
- Single patient use, not made with natural rubber latex, and not to be sterilized by any method, per the Masimo directions for use.2

Why Sample CO2 Through the Same Cannula That Delivers the Oxygen
A patient on oxygen can stop ventilating adequately and hold an acceptable SpO2 for some time, which means the oximeter reports normal while the problem is already underway. Capnography measures ventilation directly, breath by breath, and that is the argument for a cannula that carries both.
In a randomized controlled trial of 163 children undergoing 174 procedures under moderate sedation, all of whom received 2 L of supplemental oxygen by nasal cannula, blinded capnography indicated alveolar hypoventilation during 56 percent of procedures and apnea during 24 percent, while clinical staff relying on routine monitoring documented poor ventilation in only 3 percent and no apnea at all. Patients in the arm where capnography findings prompted intervention were significantly less likely to desaturate.4 In a separate controlled comparison during sedated colonoscopy, 29 episodes of apnea or hypoventilation occurred in 16 of 50 patients, and pulse oximetry detected only 38 percent of them, with a mean delay of 38.6 seconds on the episodes it did catch. The authors note that this delay is worse when supplemental oxygen is provided.5
