What SpCO and SpMet add
On the Masimo rainbow platform, SpCO is a continuous and noninvasive measurement of carboxyhemoglobin levels in arterial blood. Masimo notes that carbon monoxide exposure reduces hemoglobin's oxygen-carrying capacity and lists sources including faulty heating systems, automobile exhaust, industrial solvents, cigarette smoke and fires.
SpMet measures methemoglobin, a form of hemoglobin that is less able to bind to oxygen. Masimo lists lidocaine, benzocaine, dapsone and nitrates among drugs that may result in acquired methemoglobinemia. Readings need a monitor equipped with that parameter, and not every rainbow sensor measures both; see Masimo rainbow sensors.

- LIFEPAK 15 and 35: Stryker lists rainbow RC cables (Masimo 2406, 2404 and the 4481 EMS cable) for the LIFEPAK 15, and RD patient cables, including the 4798 RD rainbow SET RA25-04 EMS cable, for the LIFEPAK 35.
- Zoll X Series and Propaq MD: Zoll lists the rainbow RC cables, including 8000-001392 (Masimo 4481), and RD rainbow SET MD20 cables.
- Philips Tempus Pro: the manual names the Masimo 4798 EMS patient cable with the 4067 LNCS-II rainbow DCI sensor.

Flight and critical care Masimo questions
Can low oxygen saturation at altitude affect SpCO and SpMet readings?
Masimo's rainbow DCI directions list low arterial oxygen saturation levels, including altitude induced hypoxemia, among causes of inaccurate SpCO and SpMet readings, along with abnormal hemoglobin levels, low arterial perfusion and motion artifact.
Can SpO2 look normal when carbon monoxide or methemoglobin is high?
Yes. Masimo's sensor directions warn that high levels of COHb or MetHb may occur with a seemingly normal SpO2, and that laboratory analysis (CO-Oximetry) of a blood sample should be performed when elevated levels are suspected.
Is SpCO enough to decide on carbon monoxide poisoning?
No. Masimo states that SpCO is not intended to be used as the sole basis for making diagnosis or treatment decisions related to suspected carbon monoxide poisoning; it is intended to be used with additional methods of assessing clinical signs and symptoms.
Does SpMet replace a lab methemoglobin test?
No. Masimo states that SpMet monitoring with Masimo devices is not intended to replace laboratory blood testing, and that blood samples should be analyzed by laboratory instruments prior to clinical decision making.
How long can the sensor and cable run be?
Masimo's directions for its rainbow EMS patient cables give a 15 foot maximum for the sensor, cable and extension cable combination.
