Getting the Cuff Size Right in Children

Cuff size is one of the largest controllable sources of error in a pediatric blood pressure. The recommendation ties cuff selection to the patient's measured mid-upper-arm circumference, not to age or body weight: the inflatable portion should span roughly 40 percent of that circumference.1 The American Academy of Pediatrics carries the same measure-the-arm principle forward in its current clinical practice guideline for blood pressure in children and adolescents.2

The words printed on cuffs are part of the problem. A study of cuff selection found that cuffs labeled infant, pediatric, child, and small adult varied enough between manufacturers that the label alone could not tell a clinician what would actually fit, and it concluded that cuffs should be standardized, should indicate bladder size, and should be uniformly color coded.1 The same work reported that 92 percent of practitioners believed a cuff that is too small raises the reading and 55 percent believed a cuff that is too large lowers it.1
