Choosing and Using a Pediatric Chestpiece
A pediatric chestpiece is smaller so it can seal against a small chest. On an infant or young child an adult-sized diaphragm tends to span ribs and intercostal spaces rather than sit flat, and any gap lets ambient noise in. Pediatric chestpieces are sized down for exactly that reason.1 The 604 pairs a 1 3/8 inch diaphragm with a 1 1/8 inch bell.
Bell and diaphragm are not interchangeable. Acoustic bench testing of six widely used stethoscopes found that, for a given instrument, the bell and the diaphragm usually have different transmission characteristics, most noticeably at low frequencies.2 In practice the diaphragm favors higher-frequency sounds such as breath sounds and normal heart tones, while the bell favors low-frequency sounds such as some murmurs and bruits.1
One widely repeated belief is worth correcting: the bell was not designed for pediatric patients. Its purpose is low-frequency listening, and the fact that a smaller bell also fits a small chest is a convenience, not the design intent.3 That distinction is why a purpose-built pediatric chestpiece, rather than the bell side of an adult scope, is the better answer for small patients. Per the manufacturer's instructions for use, the Adscope 604 is an acoustic instrument for auscultation and does not replace clinical judgment or your agency's protocols.
